1- Information Regarding the Data Controller

Dear patients and valued visitors; this disclosure statement has been prepared by Prof. Dr. İlhan Elmacı, acting in the capacity of data controller, in accordance with the Law on the Protection of Personal Data No. 6698, in order to inform that personal data obtained within the scope of healthcare services are processed, stored, and protected with utmost care in compliance with the law and the principles of good faith.
Within this scope, personal data are processed solely for the purposes of ensuring that services are provided effectively, securely, and uninterruptedly, ensuring patient satisfaction, and improving healthcare services. Such data are not shared with third parties without explicit consent and relevant legal basis, and all necessary technical and administrative measures regarding data security are meticulously implemented.

2- Scope of Processed Personal Data

Personal data obtained through the contact form and file upload areas on the website are evaluated under the following categories:

I.Identity information (name, surname, etc.)
II. Contact information (phone number, e-mail address, etc.)
III. Request and message contents
IV. Health data (test results, reports, medical images and similar special categories of personal data)

3- Purposes of Processing Personal Data

Collected personal data are processed in a limited and proportionate manner for the following purposes:

I.Being necessary for the establishment or performance of a contract pursuant to Article 5/2 of the KVKK,
II. Processing of data relating to health and sexual life by persons under confidentiality obligation pursuant to Article 6/3 of the KVKK.

5- Transfer of Personal Data

Personal data may be shared, in accordance with the relevant legislation and within the framework of the data minimization principle, only with:

I. Authorized physicians and healthcare personnel,
II. Legally authorized public institutions and organizations.

6- Method of Collection and Legal Nature

Personal data are collected electronically, fully or partially by automated means, through digital forms and file upload areas available on the website.

7- Rights of the Data Subject

Data subjects have the following rights under Article 11 of the KVKK:

I.To learn whether their personal data are processed,
II. To request information if their personal data have been processed,
III. To learn whether personal data are used in accordance with their purpose,
IV. To request correction if personal data are incomplete or incorrectly processed,
V. To request deletion or destruction of personal data in accordance with the KVKK.
